Quality Care Measures
We are assessed by the different health plans on the quality of care we deliver to our patients.
The below quality measures are the minimum requirements that we have to demonstrate.
All patients should know about what is expected of us in their management and it is for this reason you will receive letters and calls from our office to ensure that these quality measures are being met.

All patients should know about what is expected of us in their management and it is for this reason you will receive letters and calls from our office to ensure that these quality measures are being met.
Diabetes (ages 18 - 75)
HbA1c testing
HbA1c poor control >9% LDL testing LDL Control <100mg/dl Blood Pressure <140/80 Eye Exam Kidney screening for microalbumin |
At least 2 tests per year
Most recent test should not exceed 9% At least 1 test per year Most recent LDL should be <100 Both systolic and diastolic below target eg 139/79 1 retinal exam with eye doctor per year At least one urine microalbumin test per year |
Cancer Screening and Preventive
Breast Cancer Screening (ages 40 - 69)
Cervical Cancer Screening (ages 21 - 64) Colorectal Cancer Screening (ages 45 - 75) |
One screening mammogram at least every 2 years
One screening Pap smear every 3 years Flexible sigmoidoscopy every 5 years or Colonoscopy every 10 years |
Cardiovascular Disease and Cholesterol Management (ages 18+)
Ischemic Vascular Disease LDL testing
Ischemic Vascular Disease LDL Control <100 Hypertension : Blood Pressure <140/90 |
At least once per year
Most recent LDL should be <100mg/dl Both systolic and diastolic below target eg 139/89 |
Adolescent Patients
Well Care visits in patients under 21 years of age

At least one well visit per year

Behavioral Health
Antidepressant medication management for new depression : Acute phase in ages 18+
Antidepressant medication management for new depression : Continuation phase in ages 18+ |
Patient must remain on the medication for at least 90 days (3 months)
Patient must remain on the medication for at least 180 days (6 months) |
Respiratory Conditions
Avoidance of antibiotic treatment with acute bronchitis (symptoms for <7 days)
Appropriate testing for pharyngitis (sore throat) |
Avoid antibiotic if patient has acute bronchitis
Avoid antibiotics if patient has a negative strep test or strep test is not done |
Other topics that will be addressed include:

- Bladder problems like incontinence
- Safety at home (domestic violence)
